Middle School Admissions

The following forms must be filled out in order for your child's
application to be complete. Please read the information below carefully.

APPLICATION FORM AND PARENT QUESTIONNAIRE FORM
Please read carefully and respond to each question. Forward the signed, completed form to Calvert School in the
enclosed self addressed envelope.

TRANSCRIPT RELEASE FORM
Complete and deliver the enclosed Transcript Release Form to your child's current school. This form authorizes your child's current school to send your child's complete transcript (all available report card grades and standardized test results) directly to Calvert School as soon as possible.

COMMON REFERRAL FORMS
Please have your child's current reading and math teachers complete the enclosed forms. Your child's school should mail each form directly to Calvert School in the enclosed return envelopes.

ADMISSIONS TESTING
Admissions entrance testing is required for all applicants. Calvert School accepts the ISEE test, but does not administer the test on-site. Please include Calvert School when you register for the ISEE as one of the schools to which you would like the scores sent. Calvert School may need to administer additional testing.

VISIT/SHADOW DAY
Once the Middle School Admission Office has received the application forms and school transcripts, we will call to schedule a visit for you and your child. Your visit will include a tour and an interview; your child's visit will include a "shadow day" and interview.

FINANCIAL AID
Financial aid is available to those who qualify. All awards are based on need. For more information on how to apply, visit our Financial Aid website.
